We recognize that dental visits can sometimes evoke fear or anxiety, and our team is committed to providing empathetic, personalized care to ensure every patient feels comfortable, supported, and at ease.

Please bring the following to your first appointment:

• Completed New Patient Forms
• Photo Identification
• Dental Insurance Card

If your previous dentist has taken x-rays within the past 5 years, please request that they email (info@wheatfieldfamilydentistry.com) them to our office BEFORE your appointment with us. 

If x-rays are not received before your initial appointment, a full series of x-rays will need to be taken. Insurance may not reimburse for these new x-rays. 

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at every visit.

Your first visit to our office is designed to get you better acquainted with all we offer so your future appointments are set up for success.

You will begin with a Dental Hygienist who will perform the following:

• review your dental needs and concerns
• review your health history
• measure your blood pressure
• take digital x-rays of your teeth (if necessary)
• evaluate and chart your oral and periodontal (gum and bone) health
• evaluate and chart your existing restorations 
• complete an oral cancer screening 
• professional dental cleaning (or begin periodontal treatment)
• digital wellness scan (if time allows)

The Dentist will then perform a comprehensive exam including:

• an oral cancer screening
• a tooth-by-tooth check for dental decay and infection
• radiograph evaluation
• a bite evaluation
• reviewing your dental needs and goals

Additional treatment will be scheduled for a future date.
